When a serious illness reaches a stage where comfort and presence matter most, families need more than a care plan — they need people they can count on to show up, day after day, with steadiness and warmth. Silver Lining Care Solutions provides medical companion and aide-level support that works alongside your licensed hospice or palliative care provider, so your loved one is never left to face this time alone.
What's the Difference Between Palliative Care and Hospice Care?
These two terms are often used interchangeably, but they describe different stages of care — and understanding the distinction helps families make better decisions, sooner.
Palliative care is specialized support focused on comfort and quality of life for someone living with a serious illness. It can begin at any stage of illness, even while curative treatment is still ongoing. Hospice care is a form of palliative care that begins when curative treatment is no longer the goal — typically when a physician has determined that a terminal prognosis is likely within six months. Both approaches prioritize dignity and relief over intervention.
Silver Lining works with families in both situations. Whether your loved one is managing a serious diagnosis at home or has transitioned to hospice care, our aides and companions provide the consistent, personal support that medical providers cannot offer around the clock.
What Silver Lining Provides — and What We Don't
Being clear about our scope is one of the most important things we can do for a family navigating this period.
Silver Lining is a home care registry. We place HHA-certified aides and carefully matched companions who provide personal care, comfort, and steady presence in the home. We do not provide licensed clinical hospice services — those are delivered by your hospice agency, nurses, social workers, and chaplains. What we provide is the layer of support that fills the hours between clinical visits: personal care, companionship, medication reminders, help with daily routines, and a familiar, trusted face.
Our aides work directly for your family under the registry model, which means they are matched to your loved one specifically — not rotated from a pool. That consistency matters enormously during this time.
How Our Aides Support Families During This Time
The needs of a family managing palliative or hospice care at home are different from standard home care — and our approach reflects that.
Our aides and companions provide:
- Personal care and hygiene assistance, performed with patience and gentleness
- Medication administration and administration support, including insulin injections and wound care where clinically appropriate
- Comfort-focused companionship — conversation, reading aloud, music, and simply being present
- Assistance with meals, light housekeeping, and daily routines that preserve a sense of normalcy
- Caregiver relief so family members can rest, work, or simply step away without guilt
- Live-in and hourly coverage, including overnight support and 24/7 backup if a scheduled aide cannot come in
Our network includes more than 800 vetted aides, many of whom bring medical training or clinical backgrounds from their home countries. Every aide is HHA-certified, personally interviewed, and reference-checked before joining our registry.
You Shouldn't Have to Be On Duty Around the Clock
Family caregivers in hospice and palliative situations carry an enormous weight. The desire to be present is real — but so is the exhaustion that comes from trying to provide care continuously while managing grief, work, and everything else.
Silver Lining's live-in and hourly coverage options are designed specifically for this. A live-in aide provides continuous presence in the home, allowing family members to sleep, to step away, and to show up as a son or daughter rather than a round-the-clock caregiver. Hourly coverage can be structured around the times when family cannot be present. Both options are available across Manhattan, Brooklyn, Queens, Westchester County, and Fairfield County, CT.
We have three owners, each of whom has navigated a parent's serious illness personally — oncology, Alzheimer's, Parkinson's. We built this service because we understand what families are carrying, and because we believe no one should have to carry it without support.
Questions Families Ask About Hospice and Palliative Care Support
Does Silver Lining provide hospice care directly?
No. Silver Lining is a home care registry — we place HHA-certified aides and companions who provide personal care and support in the home. Clinical hospice services, including nursing, social work, and chaplaincy, are delivered by a licensed hospice agency. Our role is to provide the consistent, personal layer of support that works alongside your hospice or palliative provider. Can a home health aide help during hospice care at home?
Yes. An HHA-certified aide can assist with personal care, hygiene, mobility, and daily routines, as well as medication reminders and, where appropriate, skilled tasks like insulin injections or wound care. This support allows your loved one to remain comfortable at home while your hospice team manages clinical care. What is the difference between hospice and palliative care?
Palliative care focuses on comfort and quality of life and can begin at any stage of a serious illness, even alongside curative treatment. Hospice care is a specific form of palliative care that begins when curative treatment is no longer the goal, typically with a terminal prognosis of six months or less. Silver Lining provides home aide and companion support for families in both situations. How is Silver Lining different from a home care agency for hospice support?
Silver Lining operates as a registry, not an agency. Your family is the legal employer of your aide, which means your aide is matched to your loved one specifically and works consistently with your family rather than being rotated from a shared pool. Registry aides can also perform skilled tasks — including medication administration and wound care — that many agency aides are not permitted to carry out.Can you provide overnight or live-in support during hospice care?
Yes. We offer both hourly and live-in scheduling, including overnight coverage. If a scheduled aide is unavailable, we provide a backup aide so there is no gap in care. This coverage is available across Manhattan, Brooklyn, Queens, Westchester County, and Fairfield County, CT.
