Sheila Kolt, LCSW, CMC

Sheila Kolt is a seasoned Aging Life Care Professional with over 20 years of experience in care management and clinical social work. She brings extensive expertise in supporting individuals with complex medical and behavioral health conditions, including dementia, stroke, spinal cord injuries, Parkinson’s disease, multiple sclerosis, oncology, and psychiatric disorders.


Sheila is the Co-founder and Co-owner of Concierge Health, as well as Principal of Silver Lining Care Solutions, a boutique direct-hire aide agency committed to connecting clients with compassionate, highly qualified caregivers who enhance quality of life through consistent, relationship-based care.

Her leadership background includes serving as Director of Care and Relationship Management at Alliance Homecare, where she developed and led the care management department. Sheila also held the role of Director of Resident Services at Brookdale Senior Living, overseeing care for nearly 200 residents. Her experience spans the entire continuum of care—from her work as a senior geriatric care manager at SeniorBridge to discharge planning at Holliswood Hospital, a private psychiatric facility. Additionally, Sheila has successfully operated a part-time private psychotherapy practice.


An active leader and fellow in the Aging Life Care Association (ALCA) since 2008, Sheila currently serves as President of the Board for the ALCA New York Chapter. In 2021, she was honored with the ALCA Excellence Award in recognition of her outstanding contributions to the field through advocacy, outreach, and service. She is also the Immediate Past President of the New York Chapter of the Case Management Society of America (CMSA), where she has been an engaged member since 2014.


Sheila holds a Bachelor of Arts in Psychology from the University of Pennsylvania and a Master of Social Work from Columbia University. Bilingual in Spanish, she is dedicated to delivering culturally sensitive care to a diverse client population.
